What Is a Cataract?

- Changing focus between distance and near when young
A healthy lens is transparent. As cataract develops, the lens may become:
- Cloudy
- Yellow or brown
- Harder
- Swollen
- Optically irregular
This causes light to be absorbed and scattered instead of forming a clear image on the retina. Cataracts normally cause slowly progressive, painless visual deterioration rather than sudden blindness.
What Does “Blindness” Mean?
Blindness does not always mean complete darkness or total absence of light perception.
The term may be used to describe profound visual impairment in which a person cannot perform ordinary visual activities even with the best available spectacle correction.
A severely cataractous eye may still detect:
- Bright light

- Macula is healthy
- Optic nerve is healthy
- Surgery and healing are uncomplicated
Not All Vision Loss Is Reversible
Vision may remain limited because of:
- Glaucoma
- Macular degeneration
- Diabetic retinal disease
- Retinal detachment
- Optic-nerve disease
- Corneal scarring
- Amblyopia
- Trauma
Advanced Cataracts Can Rarely Cause Permanent Damage
A swollen or hypermature cataract can occasionally cause:
- Phacomorphic glaucoma
- Phacolytic glaucoma
- Lens-induced inflammation
- Permanent optic-nerve damage
Childhood Cataracts Are Time-Sensitive
A dense cataract during infancy can prevent normal visual development and cause permanent deprivation amblyopia even after the cloudy lens is later removed.
A mild cataract does not always require immediate surgery.
Surgery should be considered when cataract affects:
- Driving
- Reading
- Work
- Mobility
- Independence
- Safety
- Quality of life
The cataract does not need to become “ripe,” white or nearly blind before treatment.
Sudden vision loss, severe pain, marked redness, nausea, new flashes, many floaters or a curtain-like shadow is not typical of an ordinary cataract and requires urgent assessment.
