Blepharitis: Symptoms, Causes, Treatment and Eyelid Care

Blepharitis is inflammation of the eyelid margins—the area where the eyelashes grow and where the eyelid’s oil glands open.

It is a common condition that may cause:

  • Red or swollen eyelid edges
  • Itching
  • Burning
  • Grittiness
  • Flaking or crusting around the eyelashes
  • Watery eyes
  • Dryness
  • Intermittently blurred vision

Blepharitis is usually not sight-threatening, but it can be uncomfortable, persistent and frustrating. It may also disturb the tear film and contribute to dry-eye symptoms, contact-lens intolerance and fluctuating vision.

Blepharitis is not one single disease. It may involve the front of the eyelid, the meibomian oil glands at the back of the eyelid margin, Demodex mites, bacterial overgrowth, inflammatory skin disease or several of these factors simultaneously. Treatment therefore works best when it is directed at the specific form of eyelid disease present.

The front eyelid margin contains:

The Meibomian Glands

The meibomian glands are long oil-producing glands within the upper and lower eyelids.

Their openings lie just behind the eyelashes.

The glands produce meibum, an oily substance that forms the outer layer of the tear film. This oil helps slow tear evaporation and keeps the ocular surface smooth and comfortable.

When the glands become obstructed or the oil becomes thick and abnormal, the tears evaporate more rapidly. This is known as meibomian-gland dysfunction, or MGD, and it is a major cause of evaporative dry eye.

The Tear Film

The tear film must remain smooth between blinks to provide clear vision.

Blepharitis may destabilise the tear film through:

The Main Types of Blepharitis

Anterior Blepharitis

Anterior blepharitis affects the outer front edge of the eyelid and the bases of the eyelashes.

Possible features include:

  • Crusting around the lashes
  • Flaking
  • Red eyelid margins
  • Itching
  • Lash loss
  • Misaligned lashes
  • Eyelashes sticking together
  • Soreness near the lash roots

Anterior blepharitis may be associated with bacterial overgrowth, seborrhoeic skin disease or Demodex infestation.

Posterior Blepharitis

Posterior blepharitis usually refers to inflammation related to meibomian-gland dysfunction.

  • Demodex brevis, which may inhabit deeper sebaceous and meibomian glands

Demodex mites may contribute to disease through mechanical irritation, inflammation, hypersensitivity and the carriage of microorganisms.

Mixed Blepharitis

Many patients do not fit neatly into one category.

For example, a patient may have:

  • Demodex collarettes
  • Blocked meibomian glands
  • Facial rosacea
  • Tear-film instability
  • Bacterial crusting

Treatment may therefore require several complementary approaches.

Demodex Mites and Collarettes

What Are Collarettes?

Collarettes are cylindrical or waxy sleeves of debris surrounding the base of the eyelashes.

They may appear:

  • White
  • Yellowish
  • Waxy
  • Cuff-like
  • Stuck firmly around the lash base

Collarettes at the base of the lashes are considered a characteristic clinical sign of Demodex blepharitis and can often be seen by examining the upper lashes while the patient looks downward.

Are Demodex Mites Always Abnormal?

Demodex mites can be present in people without major symptoms.

Disease occurs when the mite burden and associated inflammation become clinically significant.

Risk factors associated with Demodex blepharitis include:

  • Increasing age
  • Rosacea
  • Diabetes
  • Chronic eyelid inflammation

Not every person with a mite has symptomatic blepharitis.

Preseptal cellulitis causes more extensive eyelid redness, warmth and swelling and requires medical assessment.

Eyelid Tumours

A persistent unilateral area of eyelid inflammation can occasionally conceal an eyelid tumour.

  • Loss of lashes in one focal area
  • Ulceration

  2. Applying it over the closed eyelids.
  3. Maintaining comfortable warmth without burning the skin.
  4. Reheating it as needed.
  5. Following the duration recommended by the eye-care professional.

The device should be warm rather than painfully hot.

How Long Should a Warm Compress Be Used?

Home routines commonly use several minutes of sustained warmth.

The most important factors are:

  • Safe temperature
  • Adequate duration
  • Consistency
  • Avoiding skin injury

A cloth that cools after a few seconds may not provide sustained gland heating.

Eyelid Massage

After warming, gentle massage may help move meibum towards the gland openings.

For the upper lid, the motion is generally directed downward.

For the lower lid, the motion is generally directed upward.

Avoid:

However, detergents may irritate the ocular surface or worsen dryness in some patients. Purpose-designed eyelid cleansers may be preferable, particularly for sensitive or dry eyes.

The routine should be individualised rather than assuming that one cleaning product suits everyone.

How Often Is Eyelid Hygiene Needed?

During a flare, hygiene may be required daily or according to the treatment plan.

Once controlled, some patients continue a reduced maintenance routine to limit recurrence.

Excessive cleansing may irritate the skin, so more treatment is not always better.

Treatment of Anterior Blepharitis

Eyelid Cleansing

Regular removal of crusting and scales is often the first step.

Topical Antibiotic Ointment

An antibiotic ointment may be prescribed when bacterial anterior blepharitis is prominent.

It may be applied:

  • Along the eyelid margin
  • At night
  • For a limited treatment course

Use only the product and duration prescribed.

Antibiotic-Steroid Combination Treatment

Short courses may be used when there is significant inflammation together with suspected bacterial disease.

Steroid-containing drops or ointments require monitoring because they may cause:

  • Raised eye pressure
  • Delayed healing
  • Cataract progression in a natural lens
  • Increased susceptibility to infection
  • Worsening of certain viral or fungal diseases

Do not use leftover steroid eye medication without examination.

Treatment of Posterior Blepharitis and MGD

Home heat and eyelid care are commonly used to soften obstructed meibum and support gland secretion.

Lubricating Eye Drops

Artificial tears may improve:

  • Burning
  • Grittiness
  • Fluctuating vision
  • Screen-related discomfort

Preservative-free products may be preferable when drops are used frequently or when the ocular surface is sensitive.

Lipid-Based Lubricants

Lipid-containing drops or sprays are intended to support the oily tear layer and reduce evaporation.

The most appropriate product depends on the tear-film abnormality.

Oral Tetracyclines

Oral doxycycline or a related tetracycline may be prescribed for selected patients with:

  • Ocular rosacea
  • Significant posterior blepharitis
  • Recurrent chalazia
  • Marked meibomian-gland inflammation
  • Corneal complications

These medicines may provide anti-inflammatory effects in addition to antibacterial activity.

Possible adverse effects include:

  • Stomach upset
  • Oesophageal irritation
  • Sun sensitivity
  • Drug interactions
  • Yeast infection
  • Dental and skeletal concerns in selected groups

They are not suitable for every patient and should be prescribed by a doctor who knows the patient’s medical history.

Topical Azithromycin

Topical azithromycin may be used in selected regions or circumstances for eyelid-margin inflammation and MGD.

Availability and approved indications vary.

Anti-Inflammatory Dry-Eye Treatment

When significant ocular-surface inflammation coexists, treatment may include prescribed:

  • Ciclosporin
  • Lifitegrast where available
  • A short monitored steroid course
  • Other anti-inflammatory therapy

These treatments address associated dry-eye inflammation rather than mechanically opening blocked glands.

Meibomian-Gland Expression

In-office gland expression attempts to remove obstructed or abnormal meibum.

Evidence suggests that combining expression with home care may help some patients, but treatment effects vary and the quality of evidence is not definitive for all outcomes.

Thermal Pulsation

Thermal-pulsation systems apply controlled heat and pressure to the eyelids.

They aim to:

  • Warm obstructed secretions
  • Express the glands
  • Improve gland function

Patients should understand that treatment cannot regenerate glands that have already been extensively lost.

Intense Pulsed Light

Intense pulsed light, or IPL, applies filtered light pulses to the skin around the eyelids.

Proposed effects include:

  • Reducing abnormal superficial blood vessels
  • Modulating inflammation
  • Heating meibomian secretions
  • Improving gland function
  • Helping selected rosacea-associated disease

A 2025 systematic review of randomised trials found that IPL probably improves dry-eye symptoms compared with placebo, but the added clinical benefit over standard treatment and the certainty regarding adverse effects remain less clear.

IPL should be performed using appropriate eye protection by a properly trained provider.

Eyelid Debridement or Microexfoliation

Professional removal of eyelid-margin biofilm and debris may be used as an adjunct.

It does not necessarily treat the underlying cause on its own and may need to be combined with home care, gland management or Demodex treatment.

Treatment of Demodex Blepharitis

Confirm the Diagnosis

Treatment is more likely to succeed when Demodex disease is supported by:

  • Collarettes
  • Lash-base findings
  • Compatible symptoms
  • Microscopic confirmation when required

Lotilaner Ophthalmic Solution

Lotilaner ophthalmic solution 0.25% is a prescription anti-parasitic eyedrop approved by the United States FDA for Demodex blepharitis.

The FDA-labelled regimen is one drop in each affected eye twice daily, approximately 12 hours apart, for six weeks.

In the Saturn-2 phase 3 trial, lotilaner produced significantly higher rates of collarette cure, clinically meaningful collarette reduction, mite eradication and eyelid-redness improvement than the vehicle control after six weeks.

A follow-up study found that many treated patients retained lower collarette counts for months after completing the six-week course, although not every patient remained completely clear.

Availability and regulatory approval differ between countries.

Tea Tree Oil Products

Tea tree oil and terpinen-4-ol eyelid products have historically been used to reduce Demodex burden.

However:

  • Concentrated tea tree oil can irritate or injure the ocular surface.
  • The evidence supporting long-term effectiveness is limited.
  • Products should not be placed directly into the eye.
  • Professional guidance is advisable.

A Cochrane review found uncertainty regarding the effectiveness of tea tree oil for Demodex blepharitis because the available trials were limited and heterogeneous.

Ivermectin

Topical ivermectin applied to the eyelid area has been studied for Demodex blepharitis and may reduce collarettes and symptoms.

It is not formulated as an ordinary intraocular eyedrop, and treatment should be supervised to avoid ocular exposure or irritation.

Eyelid Hygiene Alone

Removing collarettes can improve cleanliness and comfort.

However, mechanical cleansing alone may not reliably eradicate mites living within lash follicles or glands.

Treat Associated MGD

Demodex treatment may need to be combined with:

  • Warm compresses
  • Meibomian-gland care
  • Lubricants
  • Rosacea management
  • Anti-inflammatory treatment

Makeup and Blepharitis

Should Eye Makeup Be Stopped?

During an active flare, it may be advisable to stop:

  • Mascara
  • Eyeliner

  • Cosmetic contact lenses

Makeup can trap debris, obstruct gland openings and complicate treatment.

Replace Contaminated Products

Why Can Lenses Feel Uncomfortable?

Blepharitis may cause:

Resume Only When Stable

Return to contact lenses when:

Why Does It Matter?

Blepharitis and MGD may affect:
